$250.00 each for all on site examinations.

Attorney Client Requests
10:00AM   2:00PM   4:00PM   6:00PM $900.00 each for in-lab testing with professional report of findings. Stand-by and court appearance fees will apply. On-site exams $1000.00 DFW area.
Statewide begins at $1200.00.

Counselor & Community Treatment Referrals
10:00AM   2:00PM   4:00PM   6:00PM $500.00 for initial disclosure only. Follow up monitoring or maintenance exams $350.00 if scheduled within ten months.

Pre-Sentence Polygraph Assessment

Specific Issue (Private Client/Non-Domestic Exam)

"By applying for this appointment, I am requesting a polygraph exam to be scheduled at Michael D. Park & Associates Polygraph. I understand that my credit card will be charged a $100.00 non-refundable deposit, and that I am the authorized person responsible for this credit card charge. I understand this fee is NON REFUNDABLE unless the examiner is unable to provide the examination. If I fail to keep this appointment or otherwise cancel the exam, I will forfeit this deposit. If another examiner other than an employee of Michael D. Park & Associates administers this polygraph examination, I agree to hold harmless Michael D. Park & Associates Polygraph and Michael Park, Licensed Polygraph Examiner from any action arising from the administration of this exam."
